Clinical Study on Fractures of the Shaft of the femur in Children




Abstract
Forty four children up to 15 years old with fractures of the shaft of the femur, treated in Orthopedic Department, Sacret Heart Hospital from April 1971 to March 1979, were analyzed.
We obtained the following results.
1. Fractures occured commonly in 4.10 years aged group(61%), and the incidence in. male was 1.6 times greater than that of female.
2. Traffic accident was the most common cause.
3. In classification of fractures according the site and type, middle one third and transverse fracture was the most common. The incidence of comminuted fracture was 37%, and the open fracture was 7%.
4. Common associated injuries were tibio-fibular fracture, skull fracture, pelvic bone. fracture in order of frequency.
5. Disadvantages of traction were noted as following; Reduction might not be adequate, hospitalization was long, ambulation was long and careful watching to prevent complication was required.
6. Surgical method could be chosen by prompt indication and preference and experience of the surgeon, which had some advantages-short hospitalization, early ambulation, minimal joint stiffness and make nursing care easy on multiple fractures.
7. Irani¢¥s immediate reduction and cast immobilization allowed a rapid return of the children to a familiar environment, easy nursing care,lightened financial burden and short hospitalization. The femur shaft fracture with ipsilateral tibia fl acture was treated successfully by Irani¢¥s method.
